Puppy Package
You're puppy will go home with a small puppy pack including some toys, and a well loved toy that will have been with your puppy since birth, so he/she has something familiar to go home with. A sample of their food. And you also get a packet of information about puppy care, shots, feeding, etc.
1 Year Health Agreement/Contract Example

Happy Tails Shih Tzus & Shih-Poos 1 Year Health Guarantee, Contract & Spay/Neuter Agreement
1. This agreement is between the breeder Kathryn Naughton and the said buyer _______________________________________________________________________as signed at the bottom. It is agreed between the breeder and the buyer as follows:

6. DOB: 12/27/2018
2. Conditions of sale:
1. Breeder guarantees that at the time of sale, the puppy is in good health. Should the puppy be found in poor, of a serious life threatening nature due to breeding or genetic disorder, breeder will provide buyer with a replacement puppy after the puppy is returned to breeder at the buyers expense.
2. Buyer will have the choice of a full refund or replacement puppy of similar value.
3. The following conditions apply to the sale of the aforementioned puppy:
1. $100.00 deposit is non-refundable if the buyer backs out at any time.
2. The buyer has THREE (3) business days from the time of the receipt of the puppy to have the puppy examined by a licensed veterinarian (will extend to Monday if bought over the weekend). Any request to return a dog to the breeder for a full refund must be made within this time period and may only be requested should the dog prove to have a life threatening genetic condition. Written proof of examination must be provided to the breeder. This guarantee will be terminated immediately if the dog is not examined within the allotted time.
3. The puppy is warranted free from parvovirus, distemper, hepatitis, and canine influence for a period of 5 days from the receipt by buyer.
4. Breeder cannot warrant the puppy against common conditions such as hypoglycemia, kennel cough, mites , coccidiosis, giardia, parasites nor genetic conditions that are not deemed life threatening such as hernias, mild luxating patellas (grades 1-2), cherry eye, dry eye, mild heart murmurs, allergies, undescended testicles, nor covers cherry eye repair or nasal clipping, which is sometimes found in snub nosed puppies. (Most of these conditions should be found by 8 weeks and will be discussed).
5. Under no circumstances shall the breeder be liable to the buyer or any other third party for any consequential or special damages resulting from or in a manner related to the puppy.
6. The buyer is aware of hypoglycemia and coccidia which are the two most common puppy issues at transition. Buyer is responsible to give sugar water upon arrival and force feed if needed (with syringe, canned food or cooked crumbled chicken breast). This is very important if the puppy is being shipped or travels a long distance in a vehicle.
7. Do not take the puppy anywhere for at least a week other than the vets office. Let the puppy settle into his new home. Do not cause undue stress during transitioning. This includes over stimulation by children or other dogs. Let the puppy settle in and rest!
8. Buyer will not change food for at least 4 weeks, doing so can cause your puppy to have an upset stomach.
9. Buyer is aware of additional charges for shippings, shots etc, if required. If you don't want the puppy shipped and plan on picking it up, we will hold the puppy for no extra charge, but if the puppy needs additional shots, the cost of these will be covered by the buyer.
10. At first sign of distress or illness the buyer must take the puppy to a licensed veterinarian.
11. Breeder does not warrant the coat, color, size , temperament, personality,or trainability of the puppy.
12. Upon execution of this contract, all veterinary care and it's associated costs are the sole and exclusive responsibility of the buyer.
13. If the puppy dies of a life threatening genetic disorder within the first year of life, in order for a replacement puppy, the buyer must have a necropsy performed at their state lab and report proving the genetic disease sent to the breeder . Failure to comply will receive no compensation.
14. Shipping costs are non-refundable to buyer if said puppy perishes and buyer qualifies for a refund (Shipping is the option of the buyer).
15. It is understood that at any time if the buyer can no longer care for the puppy, the puppy is to be returned to the breeder at the buyers expense. This is in the best interest of the puppy, so it will not end up in a shelter or rescue. This includes any and all circumstances, including but not limited too; Illness, death, homelessness, loss of job, moving, illness of the dog, behavioral or trainability problems.
16. Buyer agrees to neuter/spay the puppy before it reaches one year of age. Buyer agrees to supply breeder with a veterinarian's letter by certified mail as proof of a neuter/spay. If buyer fails to neuter/spay this puppy and provide proof to the breeder by one year of age, rights and ownership of the dog will automatically terminate and revert back to breeder who may immediately assume possession of the dog. Breeder's reasonable attorney's fees for recovery of the dog will be paid by the buyer. It is also understood that said puppy is being sold as a pet only not for breeding stock. If it is found that the puppy is or has been bred and produced puppies, buyer will compensate breeder a $500 fine, repayment of all money made from any puppies produced and sold. Buyer will also return the aforementioned puppy to the breeder and relinquish all ownership.
17. If the breeder should have to take the buyer to court for any part of this contract
that is not honored, the buyer will be held liable for all lawyers and court cost.
18. Our puppy families are like family to us, and because of this, we offer first pick of the next available litter should you want another puppy if your puppy dies due to a non-genetic or congenital issue (i.e. Cancer, old age, hit by a car, etc). You will still have to pay the full cost of this puppy, and all associated fees. But giving you first pick is our way of saying thank you for loving, and taking care of the previous puppy and giving him/her a happy, loving life. This is also at our discretion, should we find that you were an unfit pet parent to the first puppy, you will not be granted a second one.
4. Full and Final Agreement
1. The parties agree that this writing represents the entire agreement between them and no other representations have been made regarding the puppy described above.
2. This contract and warranty shall be governed by the laws of the State of New York
